Universal Ambulance offers emergency and non-emergency ambulance transportation services 24-hours a day, 7-days a week, 365-days per year. Transportation services are available in Macomb County, Oakland County and Wayne County, Michigan.

Schedule a Non-Emergency Transport

To schedule a non-emergency ambulance transport, please contact our Billing & Coordination Team using the form below, or by calling our Business Office at (586) 939-4350.

A valid Physician Certification Statement (“PCS”) or Certificate of Medical Necessity (“CMN”) must be submitted before a non-emergency ambulance transport can be scheduled. A PCS/CMN form must be completed by the physician or medical professional ordering the non-emergency ambulance transport.

Important! Submission of a valid PCS / CMN form is not a confirmation of health plan coverage or scheduled services. Please contact our Billing & Coordination Team to confirm health plan coverage, as well as scheduling and transport availability.

Health Insurance Plan information must be submitted to the Billing & Coordination Team to assure plan coverage. If your health plan does not provide coverage, please contact a Billing & Coordination Team member to discuss other payment options.

Health insurance plans provide different levels of coverage for non-emergency ambulance transportation services, and in many cases insurance providers may also require prior authorization for coverage to apply. Our Billing & Coordination Team can help ensure all out-of-pocket expenses and covered services are identified and communicated prior to your transport so there are no unexpected costs.
