Universal Ambulance Service offers non-emergency ambulance transportation services 24-hours a day, 7-days a week, 365-days per year. Transportation services are available in Macomb, Oakland and Wayne County, Michigan.
A valid Physician Certification Statement (“PCS”) or Certificate of Medical Necessity (“CMN”) must be submitted before a non-emergency ambulance transport can be scheduled. A PCS/CMN form must be completed by the physician or medical professional ordering the non-emergency ambulance transport.

To avoid any unexpected out-of-pocket expenses, use the form below to contact our Coordination Team. Health insurance plans provide different levels of coverage for non-emergency ambulance transportation services, and in many cases insurance providers may also require prior authorization for coverage to apply. Our Coordination Team can help ensure all out-of-pocket expenses and covered services are communicated, so there are no unexpected costs.
